The report - Supplier Responsibility - comes as Apple is trying to combat the bad press generated by a series of suicides by workers at a Foxonn factory in China, which assembles most of Apple's products.
Apple says its inspectors found 91 under-age workers at 10 factories. It cancelled its contract with one factory, which had employed 42 minors. The others suffered other penalties such as being forced to pay for the children's education, but retained their lucrative contracts.
The factories aren't named.
Apple's rules for working hours were widely broken. The rules are a 60-hour week with one day off. Only 32% of suppliers met the standard.
However, the report praises Foxconn for reacting to the suicides at its Shenzhen plant, where counsellors are now employed in a 24-hour-a-day health centre, wages have risen and nets have been put up around buildings.
